The purpose of my initial consultation is for me to evaluate your situation, and to determine whether you should file for Bankruptcy. In order to get Started, Please Bring the Following to my Office for your Consultation:



1. Get the following Bankruptcy Checklist here and print it out from your webpage. All of the instructions on this web page are included in the download. The file will open in your webpage in a common legal format called Adobe PDF Format. If You Cannot Open the File, it is because you need to Download and Install the latest version of the Free Adobe Reader here . If you have any difficulty, then please feel free to call me and I will E-mail or Mail you a copy. Make sure that you fill out the form completely.



2. One new or old bill from each creditor that you have. If you are missing a bill, then try to get the name and address and amount owed to that creditor.



3. One Paystub from each Job that you have.



4. Photo Identification and Social Security Card. If you do not have your social security card then bring another form of identification card that has your social security number, such as a medical insurance card.



5. If You own a Car or other Vehicle, bring in a document that lists the name and address of the Lienholder.



6. If you owe the IRS Taxes, then Call them at (800) TAX-1040 and order the free MFTRA (pronounced “MIFTRA”) printout for each of the years that you may owe taxes for.
